Permission Slip for those Under 18

 

Fans of all ages are invited to be a part of this project. Encouraging young people in the Arts is a good way to give them confidence and experience. Any young person under the age of eighteen years old (18 yrs old) who wishes to participate in this film MUST HAVE A PERMISSION SLIP SIGNED BY THEIR PARENTS AND HAVE A PARENT, OR LEGAL GUARDIAN,, PRESENT WITH THEM ON SET AT ALL TIMES. NO ACCEPTATIONS.

 

It is also preferred that the Project Manager meet face to face with the young person’s parent to verify their permission for their child to participate in this project and to confirm they are aware that one parent, or legal guardian, must be on the set with them at all times. No acceptations.

 

By signing this document you, the parent, acknowledges that neither the Altus Filmmakers Organization nor the Project Manager are responsible for the safety of your child. As the parent you willing sign this agreement for your child to participate in this project and agree to accompany him or her on set at all times. If you, the parent or a legal guardian, are not on set then your child will not be on set. There will be NO ACCEPTATIONS to this rule. The safety of your child is the most important thing to all of us at the Altus Filmmakers Organization.
